Academic Records

The Transcript Release Form authorizes your son’s current school to release his records to Berchmans Academy. Please complete and sign this form and give it to your son's current school.  His records will be sent directly to Berchmans Academy and are kept confidential. Records should not be uploaded by parents.

Teacher Recommendations

Applicants should submit a Math Teacher Recommendation Form and an English Teacher Recommendation Form to your son's current school.  Applicants must submit the recommendation form to your son's current homeroom teacher. The recommendation will be sent directly to the Admissions Office, and the contents will be kept confidential. By submitting the request, you accept our confidentiality agreement.
